What may be required.
- medical records related to the matter being considered;
- discharge summaries, medical letters and investigation results;
- imaging reports and other relevant specialist assessments;
- a brief chronology of events and a clearly stated question.
Please do not send medical records, a Romanian personal identification number or other sensitive data in your first message. A suitable channel will be confirmed after the preliminary review.
